In July 2024, a client with complex medical needs—including a tracheostomy, feeding tube, and inability to speak—came home for end-of-life care with Cavendish Homecare. Their initial life expectancy was just a few weeks, a stark prognosis that underscored the severity of their condition.
The client presented with a serious neck infection, a high risk of life-threatening carotid artery rupture, and required a tracheostomy for breathing and secretion management. Subsequently, due to the client’s agitation upon discharge, the Cavendish Homecare team collaborated with the multidisciplinary team. Together they developed a care plan focused on symptom management and ensuring comfort at home.
A personalised care plan was created in partnership with the medical team. It prioritised comfort, symptom management, and quality of life. The plan honoured the client’s wish to remain at home. This included medications for agitation, a syringe driver for continuous symptom relief, and specialised care for the tracheostomy and feeding tube. A dedicated team of four highly experienced Cavendish Homecare nurses provided round-the-clock care. This ensured consistency, particularly during evenings, weekends, and holidays.
The Cavendish Homecare team played a vital role in supporting the client through their final months. Their approach, grounded in core values, shaped every facet of the client’s care. Despite an initial prognosis of just a few weeks, the client lived for six months, spending their time at home in comfort, dignity, and experiencing moments of joy.
