Jake had been living with Alzheimer’s for several years. Initially mild, his condition gradually progressed and began to impact his daily routines and independence more significantly. Like many people with Alzheimer’s, he expressed a strong desire to remain at home for as long as possible, surrounded by familiar environments and close family.

Our Client’s Story…
Jake approached Cavendish Homecare as his Alzheimer’s was starting to interfere with day-to-day living, particularly with memory and awareness. He wished to remain at home, but his increasing forgetfulness made this more challenging. While he remained fully mobile, there was occasional unsteadiness. His family also reported difficulties in keeping track of his medication, along with the need for clear communication and regular reminders. Maintaining good nutrition and weight was an ongoing concern, as was reducing the risks of skin problems and infections, both of which are common in older people living with Alzheimer’s.
The Cavendish Approach…
A live-in care team was introduced to deliver a comprehensive support plan that was tailored to his medical and personal needs. Carers provided daily prompts for medication to ensure adherence and safety, and they supported him in activities such as cooking, going for walks, and completing puzzles to keep both body and mind active. Jake’s care included assistance with skin integrity, applying creams regularly to reduce the risk of leg ulcers, and encouraging a high-protein, high-calorie diet. Carers reminded him to elevate his legs when resting and monitored closely for early signs of infections, given his increased susceptibility to urinary tract and chest infections. He was prescribed Donepezil to support memory function and Lorazepam as required to manage agitation.
Family engagement was an important part of his routine. His daughter visited daily for walks, his son frequently joined him for meals, and his granddaughter often took him out to eat. These interactions were invaluable in keeping him calm and socially stimulated. Looking through photo albums with his family also proved effective in triggering memories and meaningful conversations. Personal care, such as pad changes, was carried out sensitively and discreetly, ensuring comfort and dignity.
The Outcome…
With live-in carers and strong family involvement, Jake continued to live safely and comfortably in his own home. This personalised approach not only provided the clinical oversight needed to manage his condition but also ensured that he remained engaged, reassured, and surrounded by the people and place he loves most.
