Our Client’s Story…
The need for care began when Jamie was admitted to the Royal Free Hospital’s intensive care unit. He had previously lived an active social life. Unfortunately, this was due to a coma related to his pancreatic cancer. Although he had regained consciousness, he remained bed-bound and needed highly skilled care.
His niece, Sara, who was visiting the UK, urgently contacted Cavendish Homecare. Jamie had previously shared his strong wish to stay at home, and Sara was keen to action his wishes immediately.
The Cavendish Approach…
Sara was looking for 24-hour palliative nursing support at home. Therefore, this tailored package had to address Jamie’s current medical state. He was bed-bound, confused, and needed strong pain management. Consequently, the initial assessment confirmed the requirement for round-the-clock nursing support.
The care plan demanded an extremely swift and adaptable response due to the urgency of the hospital discharge. The Cavendish Homecare response began with a rapid in-hospital assessment by a Nurse Manager, who immediately connected with Sara and the hospital’s palliative team. Following this, a team of nurses and carers was assembled to provide continuous 24-hour support. This was essential to ensure Jamie’s needs were met upon discharge. His move home was seamless. The Cavendish Homecare team managed his initial transfer and his subsequent medical needs. This included administering his prescribed pain relief, assisting with hygiene and movement, and closely monitoring his skin condition.
However, the care plan had to quickly change to match a deteriorating clinical picture. Jamie’s condition worsened rapidly. He refused food and fluids, and his nausea increased. Therefore, the Cavendish Homecare nurse quickly contacted the Palliative Team for their input. The team had to respond to a critical, urgent situation when Jamie’s condition suddenly changed for the worse. This required immediate action to manage his symptoms and ensure his comfort during the final stages of his life. In addition to providing direct care, the Cavendish Homecare team maintained continuous contact with the District Nurses and the Palliative Team. They arranged for the District Nurses to attend to begin using a syringe driver, and followed all necessary medical guidelines.
The Outcome…
The dedicated, responsive care continued until the very end. This allowed Jamie to pass away peacefully in the comfort of his own home. This case clearly shows the effectiveness of having expert nurses leading end-of-life homecare. It proves Cavendish Homecare’s capability to swiftly bring together a high-intensity team to manage a complex hospital discharge. Ultimately, the commitment and expertise of the Cavendish Homecare team allowed Jamie to fulfil his wish. He was safely discharged and received specialist cancer care at home. Crucially, his final wish to be home was fully honoured.
