What does diabetes care include?

At Cavendish Homecare, diabetes care includes a range of specialist, in-home services designed to help you manage your condition and live a full, healthy life. We focus on a tailored, patient-centred approach that addresses the unique challenges of diabetes.

Comprehensive treatment and support

A core component of our diabetes care is providing comprehensive, bespoke treatment in the comfort of your own home. Your general practitioner and healthcare team will help you find the best treatment plan for you. Our team of highly skilled nurses and carers works closely with other healthcare professionals to develop a care plan that helps you manage your diabetes and improve your quality of life. We address the physical and lifestyle challenges that arise from diabetes, ensuring a coordinated and effective health journey.

There are several types of diabetes, the most common being:

A. Type 1 diabetes

This is a chronic condition where your immune system attacks and destroys the cells that produce insulin.

B.  Type 2 diabetes

This is more common than Type 1 and is where your body either fails to produce enough insulin or the cells do not react to the insulin properly.

FAQ

Diabetes homecare allows you to manage your condition while living at home. As people get older or the condition worsens, you may require extra support with managing certain aspects of it. This can include help with ensuring a healthy diet, monitoring blood sugar, insulin injections and adhering to a medication routine. Having a carer or nurse assist you with your diabetes can alleviate any additional stressors while managing the condition.

A diabetes care plan will be tailored to your specific needs and how the condition affects you. It will usually include a summary of your condition, current treatments and treatment goals. A care plan is important as it can give you a better understanding of how to manage your diabetes and therefore, take care of your overall health.

Diabetes can cause foot problems. This is because the condition can damage the nerves in your feet, causing reduced blood supply and loss of feeling to your feet. Due to this, you could become less aware of when your foot is sore or injured. Foot care for diabetes patients and daily checks on your foot should be done to identify any potential problems early to avoid long term complications.

For people with diabetes, wounds tend to take longer to heal, or not heal well as the body is unable to produce or use insulin. Proper wound care for diabetes patients is essential to reduce the risk of infections and further complications. The most serious concern will be untreated wounds on the foot that can lead to amputation.

A diabetes diagnosis can affect someone’s daily lives as people who have diabetes will have to keep track of things such as what they eat and their blood sugar levels. You can support and take care of them by learning more about diabetes, being healthy and active together and provide emotional support. Do ensure that if you are supporting someone with diabetes daily, you are also getting enough rest and support for yourself.

All our nurses and carers are highly qualified and experienced in providing diabetes care. Our registered nurses have specific training in diabetes management, including insulin administration, blood glucose monitoring, and wound care. Our carers are also trained to assist with daily tasks, identify signs of hypoglycaemia and hyperglycaemia, and provide support with diet and exercise. We ensure all our staff receive regular, ongoing training to stay up-to-date with the latest best practices in diabetes care.
